Anyone requesting assistance can call to request a faxed application or download it from the website.
The doctor must fill out a section and sign the application.
The medication is sent one cycle at a time to the doctor's office for administration. For uninsured patients the medication is sent out before the treatment, for insured patients the medication replaced to the doctor's office.
